Chronic Disease Management

Our patients with a chronic or terminal medical condition and complex care needs may require care from a multidisciplinary team. A chronic medical condition is one that has been (or is likely to be) present for six months or longer, for example, asthma, cancer, cardiovascular disease, diabetes, musculoskeletal conditions and stroke. Our doctors are involved in the assessment the patient, agreeing management goals with the patient, identifying actions to be taken by the patient, identifying treatment and ongoing services to be provided, documenting these and including a review date in the GPMP.

Ambulatory BP Monitoring

An ambulatory blood pressure monitor is a portable blood pressure monitor that automatically records your blood pressure every 30 to 60 minutes over 24 hours during your normal daily activities including sleep.

Holter Monitoring

A Holter monitor is a machine that continuously records the heart’s rhythms. The monitor is worn for 24 – 48 hours during normal activity.

POC INR Testing

A blood test is needed to determine the effect of warfarin for each person and this is measured as the International Normalised Ratio or INR. The INR is a measure of how long it takes for blood to clot. Point-of-care testing can be defined as pathology testing performed on-site. It allows a rapid test result to be generated and used to make an immediate, informed clinical decision.

Travel Medicine and Vaccines

Our Practice provides a full range of travel medicine services which includes
Medical advice, Medications, First Aid Kits and all travel vaccines including Yellow Fever.

Paediatric Vaccines

Our fully accredited nurses provide efficient, reliable and caring immunisation for children from 6 weeks of age…and they have lollipops!

Aged Health Assessments

Our nurses offer an annual medical assessment for our elderly patients, in co-ordination with the patient’s usual GP. The information collected provides the doctor with a comprehensive overview of the patient and can assist when assessing what kind of care will best meet their needs when they are no longer able to manage at home without assistance.

Health Kids Checks

The Healthy Kids Check is an assessment of a child’s physical health, general well-being and development, with the purpose of initiating medical interventions as appropriate
Examinations and assessments include:

  • height and weight (plot and interpret growth curve and calculate BMI);
  • eyesight;
  • hearing;
  • oral health (teeth and gums);
  • toileting; and
  • allergies

 

Mental Health Assessments and Consultations

Our doctors provide thorough assessments, with a caring approach, to achieve good mental health, a sense of wellbeing, confidence and self-esteem. It enables our patients to enjoy and appreciate other people and day-to-day life.

Ante-natal Care

You can book an appointment with your doctor as soon as you know that or think you may be pregnant. At this visit, you and your doctor can discuss what type of care you would like to have, and when and where you should have your next visit.

Indigenous Healthcare

Dr Kramer has a special interest in Aboriginal Health; he attends an outreach clinic fortnightly at Yarrawarra.

ATSI Health Checks

Our nurses, in conjunction with our GPs provide comprehensive health check for Aboriginal and Torres Strait Island patients here at the practice or in the patient’s home.

Refugee Health

Our clinical staff provides thorough assessment sand consultation for refugees and other humanitarian entrants who arrive in Australia with complex and unusual medical conditions. We have access to interpreter services if needed. Dr Kramer also attends the refugee clinic at Coffs Harbour Health Campus.

Fluvax Clinic

Clinics are held in the Practice every year, commencing in late March.

Aged Care Facility Visits

Our doctors attend Woogoolga Aged Care facility weekly, to loos after our oldies.
